Northwest Colorado Visiting Nurse Association to take over Grand County Home Health patients

Northwest Colorado Visiting Nurse Association (NWCOVNA) based in Routt and Moffat counties has signed an agreement with Grand County and Grand County Home Health to provide services for the remaining 10 current long-term patients in Grand County. This agreement will be effective Feb. 13. Grand County has agreed to provide “seed” money to NWCOVNA to offset their start-up costs. The county has also agreed to provide in-kind services of office space and internet service through the end of 2016.

“I am pleased to announce that Grand County Home Health will have a transition of services to NWCOVNA at the end of the week,” said Brene Belew-LaDue Director of Grand County Home Health. “I am very relieved that our patients will be in good hands with the NWCOVNA. It has been a long process to get to this resolution and I think it is a good one for Grand County and its residents.”

NWCOVNA has been providing services to hospice patients since the Heart of the Mountain Hospice closed in early January.
