Peak Health Alliance and Elevate Health Plans announce 2025 health coverage for Grand County

The Peak Health Alliance and Elevate Health Plans by Denver Health Medical Plan are partnering again this year to offer affordable health insurance options for Coloradans in nine rural counties, according to a news release.

Open enrollment for 2025 runs through Dec. 15 for coverage beginning Jan. 1 and through Jan. 15 for plans beginning Feb. 1, the release states. With the goal of delivering accessible, budget-friendly health coverage, the partnership between the two nonprofits offers the most affordable premiums for individual health insurance plans in Grand County, according to the news release.

Individuals and families can compare rates and choose the best plan through Connect for Health Colorado or directly through Elevate Health Plans by Denver Health Medical Plan’s website.



Members in Grand County will have access to local, in-network providers, including Middle Park Health hospitals in Kremmling and Granby (and soon in Fraser), the release states.
